OPTION B: Cash Courtesy

• Total treatment fee less 5% cash courtesy paid directly to our office when treatment begins.
• The savings in clerical costs are passed on directly to you.
• This option cannot be combined with any other option.

OPTION C: Office Payment Plan

• An initial payment is due when treatment begins, with the balance paid in monthly payments.
• No finance charge.
• Past due balances are subject to a late payment fee.

OPTION D: Credit Card Pre-Authorization (Visa-Mastercard-Discover)

• Initial payment and each recurring monthly payment are automatically placed on your credit card with a single authorizing signature.
• Repay credit card at your convenience.
